Florida Hospital HospiceCare announces new admissions coordinator

Florida Hospital HospiceCare has named Maria Moak to serve as Admissions Coordinator.
In this new role, Moak will provide supervision to the Florida Hospital HospiceCare admissions team and manage the hospice referral and admissions process. In addition, she will support marketing and customer relations efforts.
Moak comes to Florida Hospital HospiceCare with nearly 20 years of experience. She began her medical career as an Emergency Medical Technician with EVAC and has experience in Intensive Care Units and Emergency Rooms.
Most recently, Moak worked for a hospice program based out of Port Orange for 12 years as a casemanger. In this role, she carried a case load of patients in the home setting, managing their symptoms and end-of-life issues. Here, she also spent four years as a patient care coordinator where she supervised a team of nearly 15 employees and coordinated the care of approximately 40 to 50 patients.
Moak, a Port Orange resident, is a Certified Hospice and Palliative Nurse and earned her Associate of Science in Nursing from Daytona Beach Community College.

Since 1996, Florida Hospital HospiceCare has cared for those in Volusia and Flagler counties facing life-limiting illnesses and injuries. With an interdisciplinary team of specialists, Florida Hospital HospiceCare can care for patients either in their own home or in one of the Florida Hospital HospiceCare inpatients units designed for acute care.
